• Application Process:Call and ask to speak to a Service Coordinator. Basic informationwill be gathered and an appointment will be arranged to begin theprocess of assessment to determine if a child is eligible.
• Eligibility Requirements:A child referred to the program will receive an evaluationconducted by a trained professional who will assess all fivedevelopmental areas (cognitive, physical, social/emotional,self-help, and language/speech.) If the child shows a delay of 25%or more (in months) in at least one of the five areas, they areeligible for services. There are additional criteria based oninformed clinical opinion and diagnosed medical conditions whichhave a high probability of resulting in developmental delay.
